Frequently Asked Questions

Why do tattoo studios need a consent form?

It verifies the client is of legal age, documents health history, records informed consent to the risks, and protects the studio from liability for undisclosed conditions.

What health conditions should be disclosed before a tattoo?

Diabetes, blood disorders, heart conditions, pregnancy, allergies, and any medication that affects healing or bleeding. The form should list these and ask the client to confirm.

Can a tattoo consent form be signed electronically?

Yes, many studios use digital consent forms. What matters is that the client reviews the risks and aftercare and that the signature is recorded.

Do minors need parental consent for tattoos?

In most states, tattooing a minor is illegal regardless of parental consent. A few states allow it with a parent present, but the safest practice is to require clients to be 18 or older.
